- Bedside Alerts: The caregiver is notified of an out of range condition BEFORE being allowed to save the measurements to the EMR. High visibility screen warnings suggest that the caregiver take alternative action (e.g., call a nurse, retake the vitals)
- Pop Up Alerts: Should the caregiver choose to save an out of range measurement, CareConnection sends an immediate notification to the MedCart Laptop and/or Nursing Desktop indicating that a breached condition has occurred and has been documented to the EMR. The nursing staff on the floor now has an immediate opportunity to intervene and redirect the care of a resident in crisis
- eMail Alerts: The DON, NPs and Physicians can opt to receive email alerting of out of range vitals for individual residents, groups or the entire census. These HIPAA secure email alerts can be picked up on cell phones when the decision makers are off campus, allowing them to direct intervention remotely.
SMART*Reports: Vitals captured through CareConnection (for auto-transmission to the EMR) are subjected to the predictive analytics of Constant Care's SMART*Technology which scrubs the daily measurements to analyze those combinations of subtle changes in condition that are indicative of potential sepsis, pneumonia, infection, dehydration and other conditions.
The DON and/or NP receives a daily email identifying those markers and their potential to escalate. Inter
vention is triggered almost before the resident even knows they are ill--certainly before any health deterioration occurs which might
require re-hospitalization. SMART*Reports clearly defines the potential diagnostic concerns which are revealed by the trended
presence of these combinations of changes in condition. SMART*Reports also serve as a QA "improvement opportunity" tool that
can help your team avoid survey deficiencies!
