They made good even better! Upgrading to Constant Care's new, ruggedized Vitals CareCart has been a cost effective move for us. Our team says the new vitals monitor is more accurate, easy to use and the new tablets sync with our AmericanHealthTech EMR software so easily. As a result, we are taking even more vitals and improving our quality of care. At the same time we are saving CNA time and putting efficiency in our bottom line equations. Thanks to CCT for improving on an already great product for us.”
- The SMART*Report is a great tool that allows us to monitor a patient and prevent possible infection. I look at it daily and I see the benefit to identify any underlying things going on, even if a patient appears to be okay on the outside.
We avoided a readmission today! The SMART*Report was sent to my email at 6:30 am this morning and indicated a couple of slightly elevated vitals markers for one of my residents. The resident was asymptomatic but we checked on her because of the report. Her lungs sounded clear but she was having a bit of trouble taking a deep breath. We ordered a chest x-ray which was positive for infiltrate and she was diagnosed with pneumonia. We were able to identify the problem and start treatment before it was too late. With our fragile population, a few hours can make a big difference. CareConnection's SMART*Reports gave us the edge.
Our team is serious about using technology to improve care - - and CareConnection does just that! During training we talked about saving minutes a day - maybe it adds up to a few extra minutes visiting a resident or maybe it's a bathroom break for a busy nurse. When efficiency saves time that can be used to improve the resident and team member experience, our leadership team is committed to spending the resources.
Everyone, including our discharge planner, is in love with the web reports. Being able to track the SPO2 live and communicating that information without superfluous info is what our Medicare DME providers need. His comment: "That is amazing"
"Using Constant Care Technolgy's CareConnection® platform, our certified nursing assistants are now able to upload vital signs from residents automatically into PointClickCare, our electronic health record system. That saves nursing time and reduces errors plus the alert system helps prevent readmissions. There’s no risk any longer---and we can free up our nurses to work at the top of their licenses since we have confidence in the CNA's ability to handle vitals capture with the help of the CareConnection® system.
We have seen our documentation compliance go to 100 percent almost every single day since CNAs are more inclined to document episodes more frequently. CareConnection's CareHub Mobile Kiosk design gives us in-room point-of-care documentation and reduces the risk of a CNA forgetting to mention things like how a resident’s eating pattern has changed. This system has greatly improved team morale and we have less turnover. It’s made the staff’s job easier."
"The implementation went very well. The trainer was excellent. She opted to train our users in small groups which allowed them to learn much better. I heard nothing but positives from our staff. The only disappointment that they have is that we ordered only 3 of the Vitals CareCarts. They just love the new machines and especially the digital wheelchair scale. Overall, it has been a positive experience. Our staff is happy because it saves them time from doing manual entries, and the management has the peace of mind that the vitals entries are the actual numbers."
"Constant Care Technology has been working with us here at Cognos Consultants since February 2014. We have come to rely greatly on the quick and accurate input of vitals for our residents that instantly become available to nurses in our EMR, American HealthTech. They have been there with us every step of the way and continue to assist us with maintaining usage and driving down errors in vitals. Constant Care has been not just a simple provider of a service but a valued partner helping us to achieve bottom line savings and better resident care."
