We avoided a readmission today! The SMART*Report was sent to my email at 6:30 am this morning and indicated a couple of slightly elevated vitals markers for one of my residents. The resident was asymptomatic but we checked on her because of the report. Her lungs sounded clear but she was having a bit of trouble taking a deep breath. We ordered a chest x-ray which was positive for infiltrate and she was diagnosed with pneumonia. We were able to identify the problem and start treatment before it was too late. With our fragile population, a few hours can make a big difference. CareConnection's SMART*Reports gave us the edge.
